Job summary

Enhabit Home Health & Hospice is seeking a dedicated Physical Therapist to assess and treat patients in home health settings. You will employ physical modalities to improve mobility, independence in daily living, and self-care, following physician orders and the plan of care under the Branch Director's supervision.

The role requires a valid state license, reliable transportation, and the ability to drive in various weather conditions to deliver skilled services to patients in their homes.

Qualifications

• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.
• Must wear seatbelts at all times while driving.
• Must have dependable transportation kept in good working condition.
• Must be able to drive an automobile in all types of weather conditions.
• Must be currently licensed in the state of employment.

Responsibilities

• Provide skilled physical therapy to patients in home health settings under physician orders and established plan of care, with supervision as needed.
