Job summary

Advanced Home Care & Hospice of Northern Idaho seeks a licensed Physical Therapist to join our Lewiston team on a PRN, weekdays schedule. You will evaluate patients at home, design personalized therapy plans, and implement treatment to improve mobility and function.

Strong communication with patients, families, and the interdisciplinary team is essential. Candidates with home health or hospice experience are preferred but not required.

Qualifications

• 2+ years of physical therapy experience, preferably in home health or similar settings.
• Excellent communication and interpersonal skills with patients and families.
• Ability to prioritize tasks and adapt to a fast-paced environment.

Responsibilities

• Conduct thorough evaluations and develop individualized therapy plans.
• Implement treatment programs to improve mobility, strength, and function.
• Educate patients and families on exercises and home programs.
• Document progress and communicate with the healthcare team.
• Collaborate with physicians and other professionals to coordinate care.
