Job summary

Choice Health At Home is looking for a Physical Therapist Assistant (PTA) to support licensed physical therapists in delivering rehabilitative care in a Tyler, TX home health setting. The PTA will implement treatment plans, monitor progress, and adapt activities to maximize recovery and independence.

The role requires an accredited PTA program, state licensure, CPR, and a preference for 1 year of home health experience.

Qualifications

• Accredited Physical Therapy Program completed.
• State licensure exam passed.

Responsibilities

• Assist licensed physical therapists in implementing treatment plans.
• Conduct therapeutic exercises and modalities under supervision.
• Monitor patient responses and document progress for the supervising PT.
• Educate patients and families on exercises and use of assistive devices.
