Qualifications

• Active Physical Therapy license in the state of employment.
• Excellent patient care and interpersonal skills.
• Willingness to travel to patient homes as part of field-based role.

Responsibilities

• Assess and treat patients with physical impairments using physical modalities.
• Improve mobility, functional abilities, and daily living skills.
• Educate patients and families; implement plan of care under physician orders and director supervision.
