Job summary

UnitedHealth Group's LHC HomeCare of South Alabama seeks a Home Health Physical Therapist in Alabama. You will assess patient needs, develop and implement a PT plan of care, and document progress during home visits with respect to mobility, endurance, and pain.

Responsibilities include evaluating functional status, collaborating with nurses and physicians, and educating patients and families about the plan of care. A valid PT license and driving access are required, with CPR preferred.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
• Preferred: CPR certification or ability to complete within 90 days of hire

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Assists in development of the therapy plan of care.
• Provides services within the scope of practice per state laws and plan of care.
• Observes and reports patient response to treatment and changes in condition.
• Treats patients to relieve pain and restore function.
• Instructs patient and family about plan of care, limitations, and progress.
• Completes progress notes and PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and frequency of care.
