Qualifications

• Current Physical Therapy licensure in Alabama (state).
• Driver's license and vehicle insurance or access to reliable transportation.

Responsibilities

• Evaluate patients' functional status and therapy needs.
• Provide PT services per plan of care and state guidelines.
• Observe patient responses and report changes to supervising nurse/physician.
• Treat to relieve pain, restore function, and maintain maximum functioning.
• Educate patient and family about plan of care and progress.
• Complete visits and progress notes per policy.
• Develop PT plan of care within state guidelines.
• Report outcomes and frequency of care.
