Job summary

VitalCaring Group is seeking a Home Health Physical Therapist to help patients regain independence in the comfort of their homes. You will deliver patient-centered care, evaluate functional status, and collaborate with physicians and the care team to design individualized plans.

Qualifications

• Required: graduate of an accredited Physical Therapy program.
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience preferred.
• Current CPR certification and valid driver’s license.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Document patient care and progress accurately and timely within the EMR.
• Collaborate with nurses, therapists, and physicians for coordinated care.
