Qualifications

• Minimum 1–2 years of experience in home health preferred.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate and assess patients’ functional status and mobility.
• Develop and implement individualized care plans.
• Monitor patient progress and adjust treatment plans as needed.
• Supervise Physical Therapist Assistants (PTAs) according to state guidelines.
