Responsibilities

• Provides physical therapy services to residents under the direct supervision of the physical therapist.
• Identifies indications for evaluations at appropriate intervals and assists in the evaluation under the direction of the physical therapist.
• Assists in identifying a target list, short-term goals, long-term goals and treatment plan.
• Conducts individual treatment as assigned by the physical therapist and outlined in treatment plan, including muscle strength, mobility, balance, gait, ROM, and modalities.
• Assists in selecting physical therapy techniques/media and sequencing activities, adapting as indicated by resident performance and consulting with the physical therapist.
• Monitors patient response to intervention and consults with the physical therapist if modification is needed.
• Helps establish and modify individual patient schedules with the physical therapist and other team members.
• Adheres to department and patient schedule and adjusts as needed for treatment regimen.
• Maintains acceptable patient care ratios as determined by the physical therapist.
• Provides, recommends, and fabricates adaptive devices or other equipment; trains patient and family in its use.
• Participates in patient, family and staff education.
• Confers with team members to coordinate the total rehabilitation program.
• Adheres to Northwoods Rehab documentation procedures for physical therapy services.
• Delivers treatment per established standards of practice and department procedures.
• Maintains a safe and clean work area and follows infection control and OSHA standards.
• Demonstrates sound judgment in planning, implementing and following up on therapy programs.
• Maintains positive relationships with coworkers, supervisors, patients, families and community personnel.
• Reports to work on time and coordinates schedules for efficient treatments.
• Assists with and participates in Quality Assurance programs as required.
• Adheres to confidentiality standards of the patient and organization.
• Projects a positive and professional image at all times.
• Maintains passwords for software required for position and keeps them secure.
• Performs other duties as assigned to discharge responsibilities effectively.
