AccentCare, Inc. in Oakdale, MN is seeking a Licensed Physical Therapist Assistant to join our home health team.

You will provide patient care under the supervision of a licensed Physical Therapist, educate patients and families, document care, and participate in the interdisciplinary team to meet productivity standards. The role covers MN and WI service areas (MN: Bayport, Lakeland, St Paul; WI: Hudson, River Falls) with dual MN/WI licensure.

Qualificações

• Associate degree in Physical Therapy (required).
• CPR certification required.
• Valid driver's license and ability to travel to all locations.
• Strong documentation and patient education skills.

Responsabilidades

• Provide patient care and document treatment per physician orders and policy.
• Maintain accurate daily notes and treatment records.
• Educate patients and families and communicate findings to supervising therapist.
• Collaborate with the interdisciplinary team and participate in meetings.
