Job summary

Ambercare Homecare in New Mexico is seeking a dedicated Physical Therapist Assistant to join our growing team in Santa Fe and Espanola. This is a full-time position with a Monday–Friday 8am–5pm schedule in patients’ homes.

You will provide direct PT services under a licensed PT, perform therapeutic exercises, educate clients, and document progress in the clinical record. Strong CPR and home-visit capabilities are required; reliable transportation is essential.

Qualifications

• Graduate of an accredited PTA program with an Associate’s degree approved by APTA.
• Current licensure as a Physical Therapist Assistant in the state of practice.
• At least one year of PTA experience (preferred).
• CPR certification current and maintained.
• Ability to make home visits and provide nursing care in the home environment.
• Reliable transportation, valid driver’s license and auto insurance.

Responsibilities

• Provides direct physical therapy per directions of a qualified PT and plan of care.
• Treats patient to relieve pain, improve function, and maximize performance.
• Guides patients in active and passive therapeutic exercises and gait training.
• Administers therapeutic modalities as appropriate for stable conditions.
• Instructs and motivates patients to learn and perform functional activities.
• Reports patient progress and responses to treatment to the supervising PT.
• Prepares clinical notes and progress reports for the patient record.
• Notifies supervisor of changes in patient condition and plan of care as needed.
• Instructs patients on use of mobility devices and prosthetics.
