Job summary

Ambercare Homecare in Santa Fe, NM is seeking a licensed Physical Therapist Assistant for full-time work Monday through Friday, 8am–5pm. The role involves providing direct PT services in patients' homes across 87501–87508 with travel required.

Responsibilities include implementing the plan of care, administering modalities, and documenting progress for review by a supervising PT. A valid NM PTA license and CPR certification are required.

Qualifications

• Graduate of an accredited Physical Therapy Assistant program.
• Current licensure as a Physical Therapist Assistant in NM.
• At least 1 year of PTA experience preferred.
• Current CPR certification.
• Ability to travel to homes for care.

Responsibilities

• Provide direct physical therapy per the supervising PT and plan of care.
• Treat patients to relieve pain and restore function.
• Assist in active and passive therapeutic exercises and gait training.
• Administer therapeutic modalities as prescribed.
• Instruct and motivate clients in functional activities.
• Observe progress and report to the PT.
• Prepare progress notes and document treatments.
• Report changes to supervising PT and Director of Nursing/therapy.
• Instruct patients in use of mobility aids.

What You’ll Do

• Provide direct physical therapy according to directions of a qualified physical therapist and in accordance with the physician's plan of care and as defined in the state Physical Therapy Practice Act.
• Treat patients to relieve pain, develop or restore function, and maintain maximum performance.
• Direct and aid patients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities, and prosthetic training.
• Administer non‑complex therapeutic exercises, therapeutic massage, traction, heat, light, cold, water and electrical modalities to clients with relatively stable conditions.
• Instruct, motivate and assist clients in learning and improving functional activities.
• Observe client progress and response to treatment and report findings to the physical therapist.
• Assist in preparing clinical notes and progress reports documenting physical therapy treatments performed and submit them for review into the clinical record.
• Report any changes in the client’s condition to the qualified supervising Physical Therapist and Director of Nursing/Director of Therapy. Changes in the plan of care may be made following authorization, instruction and supervision by the Physical Therapist.
• Instruct patients in care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.

Qualifications

• Graduate of an accredited physical therapy assistant program with either an Associate’s degree in science or applied science approved by the American Physical Therapy Association.
• Current licensure as a Physical Therapist Assistant in the state of practice.
• At least one (1) year’s experience as a Physical Therapy Assistant (preferred).
• Current CPR certification.
• Ability to make home visits and provide nursing care in the home environment.
• Must have reliable transportation, current driver's license and appropriate automobile insurance.
