Assured Home Health, part of the LHC Group and within the Optum family, seeks a Physical Therapist Assistant to implement treatment plans and educate patients to improve well-being in home settings. You will follow the supervising PT's plan and contribute to high-quality, compassionate care.

The role requires Oregon licensure, weekend availability, and a valid driver’s license with reliable transportation. Pay ranges reflect responsibilities and productivity in a home health environment.

Formación

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of Oregon
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
• Available to work a weekend rotation

Responsabilidades

• Follow all state laws governing the provision of physical therapy in home care and perform only those procedures defined by the supervising PT; propose changes to the plan of care as needed.
• Instruct and assist patients in active and passive exercise, muscle re-education, gait, functional activities, ADL, transfers, safety, and prosthetic training.
• Observe, record, and report patient responses and changes in condition to the supervising PT, nurse supervisor, and physician; coordinate care with the health care team.
• Provide care to patients in diverse home settings regardless of age, race, creed, color, sex, disability, or financial condition.
