Job summary

Assured Home Health, a part of LHC Group, is seeking a Physical Therapist Assistant to deliver treatment and education under the plan of care established by a Physical Therapist.

Under supervision, you will guide patients through exercises, monitor progress, document responses, and support families. A valid Oregon PTA license and reliable transportation are required.

This role offers a $7,500 sign-on bonus for external candidates and a comprehensive benefits package.

Qualifications

• Current Oregon PTA license to practice in home health.

Responsibilities

• Follow state laws and plan of care under the supervising Physical Therapist.
• Instruct and assist patients in active and passive exercises, gait, ADLs, and safety.
• Observe and report patient responses and changes in condition to the supervising PT/nurse/physician.
• Educate patient, family, and caregivers to participate in rehabilitation activities.
• Document progress notes for each visit and contribute to the Plan of Treatment.
