Vantage Health Partners is seeking a Home Health Physical Therapist to join our on-site team in Parker, Colorado. This full-time role serves patients in home health and assisted living settings across Parker, Castle Rock, and Castle Pines, with day shifts and opportunities for growth.

You will conduct comprehensive evaluations, develop individualized plans, and deliver therapeutic exercise, mobility, strength, balance, and coordination interventions to improve function and independence.

Qualifications

• Colorado PT license (current)
• Graduate of accredited PT program (BS, MS, MPT, or DPT)
• Experience with patient evaluation and care planning
• Experience with therapeutic exercise and mobility training
• Competency in strength, balance, and coordination interventions
• Skilled in progress monitoring and clinical documentation
• Preferred: 1 year PT experience
• Preferred: Home health or assisted living experience
• Preferred: Wellness program design
• Strong communication and interpersonal skills and teamwork

Responsibilities

• Conduct comprehensive physical therapy evaluations in home health and assisted living settings
• Develop, implement, and modify individualized patient care and treatment plans
• Deliver therapeutic exercise and mobility training to restore function and independence
• Design and apply strength, balance, and coordination interventions to reduce fall risk
• Monitor patient progress and adjust interventions based on clinical outcomes
• Document evaluations, treatment sessions, and progress notes accurately and timely
• Educate patients and caregivers on home exercise programs, safety, and injury prevention
• Collaborate with physicians, nurses, and other interdisciplinary team members to coordinate care
• Manage caseload while traveling across Parker, Castle Rock, and Castle Pines
• Maintain compliance with regulatory, documentation, and home health standards

Description

We are seeking a Home Health Physical Therapist to join our team in Parker, Colorado. This full-time, on-site position offers the opportunity to deliver high-quality, patient-centered physical therapy care in home health and assisted living settings across Parker, Castle Rock, and Castle Pines, CO. This is a day shift role, with full-time and part-time permanent positions available.

As a Home Health Physical Therapist, you will conduct comprehensive patient evaluations, develop individualized care plans, and deliver therapeutic exercise, mobility, strength, balance, and coordination interventions that help patients regain independence and improve quality of life. You will work directly with patients in their homes and assisted living communities, monitoring progress and documenting outcomes to support continuity of care.

Requirements

• Current Colorado Physical Therapist license
• Graduate of an accredited Physical Therapy program with a BS, MS, MPT, or DPT degree
• Proficiency in patient evaluation and care planning
• Experience with therapeutic exercise and mobility training
• Competency in strength, balance, and coordination interventions
• Skilled in progress monitoring and clinical documentation
• Preferred: At least one year of physical therapist experience
• Preferred: Home health or assisted living experience
• Preferred: Experience in wellness program design
• Preferred: Strong interdisciplinary collaboration skills
• Preferred: Strong communication and interpersonal skills
