Under the supervision of a Physical Therapist, you will deliver therapeutic exercise, mobility training, and use assistive devices, while documenting progress and maintaining safety and quality care.

Qualifications

• Graduate of an accredited PTA program.
• Current Colorado Physical Therapist Assistant license.
• PTA certification.
• Valid driver's license.
• Proficiency in therapeutic exercise instruction.
• Ability to use mobility devices such as walkers and canes.
• Knowledge of stretching and massage techniques.
• Ability to observe, document, and report patient progress to the PT.
• Preferred: At least 1 year of PTA experience.
• Preferred: Home health or assisted living PTA experience.
• Preferred: Strong attention to detail, compassion, and empathy.
• Preferred: Good interpersonal and communication skills.

Responsibilities

• Carry out physical therapy treatment plans developed by the supervising Physical Therapist.
• Deliver therapeutic exercise instruction tailored to individual patient needs.
• Train patients in the safe use of mobility devices such as walkers and canes.
• Apply stretching and massage techniques to improve mobility and comfort.
• Observe and monitor patient progress throughout each treatment session.
• Document treatment provided, patient response, and progress accurately and timely.
• Communicate patient status and concerns to the supervising Physical Therapist.
• Provide patient and family education on home exercise programs and safety.
• Travel between home health and assisted living locations across Golden and Lakewood.
• Maintain a safe, professional, and patient-centered approach in all home environments.
• Follow all clinical protocols, regulatory requirements, and company policies.

Description

We are seeking a dedicated Home Health Physical Therapist Assistant to join our team on-site in Lakewood, CO. This full-time, permanent position offers the opportunity to deliver compassionate, hands-on rehabilitation care to patients in home health and assisted living settings across Golden and Lakewood, Colorado. This role plays a vital part in helping patients regain mobility, independence, and quality of life in familiar environments.

As a Home Health Physical Therapist Assistant, you will work under the direction and supervision of a Physical Therapist to carry out individualized treatment plans. You will provide therapeutic exercise instruction, mobility training, stretching and massage techniques, and guidance on the use of assistive devices, while closely observing patient progress and documenting findings for the supervising therapist. Schedules include weekdays with occasional weekend availability.
