Residential Home Health and Residential Hospice is seeking a Home Health Physical Therapist to deliver skilled care to patients in their homes in Lehighton, PA. This role offers autonomy, meaningful patient relationships, and opportunities for growth.

You will evaluate patients, design therapy plans, educate families, and document care in the EMR. A driving role with ~20% travel and a $10,000 first-year bonus await the right clinician.

Qualifications

• Active Physical Therapist, PT license in the state you work.
• One year of experience as a Physical Therapist.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory
