Job summary

Residential Home Health and Hospice is seeking a Physical Therapist (PT) to provide skilled home health care in the Lehighton, PA area. You’ll work autonomously within an interdisciplinary team, traveling to patient homes to deliver evaluations, develop therapy plans, and educate patients and caregivers.

Active PT license in your state and at least 1 year of PT experience are required. A valid driver’s license is needed for ~20% travel.

Qualifications

• Active Physical Therapist license in the state you work.
• Minimum one year of PT experience delivering quality patient care.
• Valid driver’s license with ~20% travel to patient locations.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team.
• Perform comprehensive patient evaluations and develop individualized therapy plans.
• Educate patients and caregivers on exercises, injury prevention, and safe mobility at home.
• Maintain thorough EMR documentation and ensure regulatory compliance.
• Travel to patient homes within the assigned territory.

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you

• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence

• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home

• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

• Work independently, but never alone

• Travel to patient homes within assigned territory

PT Qualifications

• Active Physical Therapist, PT license in the state you work

• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care

• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
