Job description

BAYADA Home Health Care is currently seeking an experienced Physical Therapist (PT) for full-time, part-time, or per diem opportunities providing home health visits. This position services adult and geriatric clients in Carbon County, PA.

As a home care Physical Therapist, you will be a vital part of a multidisciplinary team offering rehabilitative care and skilled nursing to help clients remain at home.

Responsibilities include:

1. Make home visits within designated territories.
2. Perform diagnostic tests such as mobility, joint range of motion, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
3. Implement protective interventions to prevent wound formation and promote healing using evidence-based dressings and topical agents.
4. Develop and update individualized care plans, including manual exercises, gait training, and balance interventions.
5. Assess and revise care plans regularly, collaborating with nursing staff as needed.
6. Educate clients and family members on rehabilitative activities to promote health and independence.
7. Document observations, interventions, and evaluations accurately using a tablet-based system.

Qualifications include:

• Current Pennsylvania Physical Therapist (PT) license.
• Graduation from an American Physical Therapy Association-accredited program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; experience with electronic medical records (EMR) preferred.
