The Physical Therapist in Home Health is responsible for the assessment and evaluation of patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the Physical Therapist works to determine a treatment plan, performs interventions aimed at improving and enhancing the patient's well-being, and evaluates the patient's progress.



Responsibilities

  • Evaluate the patient's functional status, status of all body systems as required for CMS documentation, and physical therapy needs. Consult with the physician in the development of the therapy plan of care.

  • Provide services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with other healthcare team members.

  • Observe and report the patient's response to treatment and any changes in condition to the nurse supervisor and/or physician.

  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using accepted standards and evidence-based techniques.

  • Instruct and inform healthcare team members, the patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.

  • Visit patients within the entire service area of their agency and nearby LHC agencies as needed.

  • Complete a progress note for each visit according to Plan of Treatment and submit it per policy.

  • Complete all patient evaluations and develop the PT plan of care within state guidelines. Report outcomes, goals, and projected care frequency to the nurse supervisor within 24 hours of evaluation.

  • Participate in staff conferences and committees as requested.

  • Supervise home health aides and PTAs as allowed by policy and state regulation.

  • Participate in weekend rotations to provide services as needed.

  • Prepare discharge summaries and periodic reports on patient's condition.

  • Provide in-service education as requested.

  • Adhere to LHC Therapy Standards of Practice.

  • Maintain current clinical knowledge and licensure.

  • Perform other duties as assigned.



Education and Experience

Licensure Requirements



  • Current Physical Therapy licensure in the state of practice

  • Current CPR certification

  • Valid driver's license, vehicle insurance, and access to a dependable vehicle
