The Physical Therapist in Home Health is responsible for the assessment and evaluation of patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the Physical Therapist develops a treatment plan, performs interventions to improve the patient's well-being, and evaluates progress.

Responsibilities
  • Evaluates the patient's functional status, body systems, and physical therapy needs. Consults with the physician for the development of the therapy plan of care.
  • Provides services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team.
  • Observes and reports the patient's response to treatment and any condition changes to the nurse supervisor and/or physician.
  • Treats patients to relieve pain, develop and restore function, and maintain maximum functioning using evidence-based techniques.
  • Instructs and informs the healthcare team, patient, and family/caregivers regarding the plan of care, limitations, and progress.
  • Visits patients within the service area and nearby agencies as needed.
  • Completes progress notes for each visit and submits them per policy.
  • Completes evaluations and develops the PT plan of care within state guidelines. Reports outcomes, goals, and projected care frequency within 24 hours of evaluation.
  • Participates in staff conferences and committees as requested.
  • May supervise home health aides and PTAs as per policy and regulations.
  • Participates in weekend rotations as needed.
  • Prepares discharge summaries and patient condition reports at least every 60 days.
  • Provides in-service education as requested.
  • Adheres to therapy standards of practice.
  • Maintains up-to-date clinical knowledge and licensure requirements.
  • Performs other duties as assigned.
Education and Experience

Licensure Requirements

  • Current Physical Therapy licensure in the state of practice
  • Current CPR certification
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle
