Job summary

CenterWell Home Health is seeking a Home Health Occupational Therapist Assistant to deliver therapy services under the supervision of a qualified Occupational Therapist. You will implement treatment plans, design adaptive equipment, and support patients and families in home-based rehabilitation to restore strength and independence.

The role requires OTA licensure, CPR certification, and a valid driver’s license with reliable transportation for home visits.

Qualifications

• Current and unrestricted OTA licensure.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation.

Responsibilities

• Provide therapy services planned, delegated and supervised by the Occupational Therapist.
• Assist in implementing vocational/education programs and activities established by the OT.
• Design/adapt equipment and living environments to support function.
• Fabricate devices to assist and improve independence.
• Provide therapeutic treatment and instruction as directed by the OT and physician orders.
• Document observations in patient records and prepare clinical notes.
