Job summary

CenterWell Home Health seeks a Home Health Occupational Therapy Assistant to deliver patient-centered therapy in home settings under the supervision of a licensed Occupational Therapist. You will contribute to patient goals by restoring strength, mobility and independence using functional rehab plans.

Qualifications

• Current and unrestricted OTA licensure.
• Current CPR certification.
• A valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Provide therapy services planned, delegated and supervised by the Occupational Therapist.
• Assist in implementing vocational/education programs and activities.
• Design/adapt equipment and living environments to restore, reinforce and enhance task performances.
• Provide therapeutic treatment and instruction per physician orders and OT guidance.
• Record information in patient records and assist with progress reports and documentation.
