Job summary

PruittHealth at Home is seeking a licensed Occupational Therapist (OT) to provide evaluative, consultative, and treatment interventions for home health patients in their residences. You will assess, document, and contribute to care plans while coordinating with the physician and interdisciplinary team.

The role emphasizes timely documentation (OASIS, admissions, recertifications) and maintaining compliance.

Qualifications

• Must have a valid and unrestricted professional license in state of practice.
• Graduate of ACOTE accredited school.
• If foreign-trained, program must be deemed substantially equivalent by a credentials evaluation organization approved by AOTA.

Responsibilities

• Assesses, monitors, documents and reports progress of patient health and condition using required documents via clinician preferred technology, within established timelines.
• Collaborates with interdisciplinary team and physician to develop, integrate and manage the plan of care.
• Demonstrates competency in home care specific patient care skills and prioritizes patient needs.
• Completes documentation timely and accurately at point of care (OASIS, SOC/Admissions, Recertifications, ABN, etc.).
• Maintains license, certification and clinical records to meet regulations.
