Job summary

PruittHealth is seeking an Occupational Therapist to provide evaluative, consultative, and treatment interventions to home health patients in their residences. The role emphasizes collaboration with the interdisciplinary team, accurate documentation, and adherence to licensure and accreditation standards.

Qualifications

• Must have a valid and unrestricted professional license in state of practice.
• Graduate of ACOTE accredited Occupational Therapy program.
• If foreign-trained, equivalence must be demonstrated by a credential evaluation organization approved by AOTA.

Responsibilities

• Assesses, monitors, documents and reports patient progress using required documents and clinician-facing technology.
• Collaborates with the interdisciplinary team and physician to develop and manage the plan of care.
• Demonstrates competency in home health care and discipline-specific skills.
• Responds to priorities in patient care with initiative and sound judgment.
• Assists with training and orienting others.
• Utilizes teaching materials for patient/family education and completes documentation timely (OASIS, SOC/Admissions, Recertifications, ABNs, visits).
• Adheres to compliance programs and agency policies; meets productivity standards.
• Participates in surveys, in-services, and case conferences.
• Maintains current license, certification, and records to meet regulations.
