Job summary

Optum, in partnership with Caretenders within the LHC Group, is seeking an Occupational Therapist in Home Health to assess and plan care for patients in their homes. The role features a competitive sign-on bonus, a compensation structure combining per-visit and hourly pay, and comprehensive benefits.

You will evaluate functional status, perform ADL assessments, and coordinate with the care team. Qualified candidates hold OT licensure, CPR, and have reliable transportation to service diverse home

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Access to a dependable vehicle or public transportation.
• Ability to work in diverse home environments.

Responsibilities

• Provides OT services within the scope of practice as defined by state laws and plan of care.
• Evaluates functional status and CMS-related documentation; consults with the physician on care plans.
• Observes and reports patient responses to treatment to nurse supervisor and physician.
• Conducts patient assessments of functional ability using standard tests.
