VitalCaring Group is seeking a Home Health Occupational Therapist to help patients regain independence and navigate care at home. You will work closely with patients, families, and interdisciplinary teams to deliver personalized, high-impact care in a field-based setting.

The role involves evaluating functional status, ensuring home safety, and developing measurable treatment goals. OC/CPR certified with driving ability preferred.

Qualifications

• Graduate of an accredited OT program.
• NBCOT certification required.
• Current state OT licensure.
• Minimum 1 year clinical experience preferred (home health a plus).
• Current CPR certification.
• Valid driver’s license and auto insurance.

Responsibilities

• Evaluate patients’ functional status and daily activities.
• Assess home environments for safety risks and modifications.
• Develop individualized treatment plans with clear goals.
• Provide therapeutic interventions to restore function.
• Fabricate splints and train on adaptive devices as appropriate.
• Support patients returning to work with evaluations.
• Educate patients and families on care plans and safety.
• Coordinate services with care team for seamless care.
• Participate in discharge planning and transitions.
