Job summary

Choice Health at Home is seeking an Occupational Therapist (OT) for our Florida Clearwater home health branch. This full-time role requires licensed OT, CPR, and a valid driver’s license to serve patients in their homes across Clearwater, FL.

The OT will assess needs, develop Plans of Care, document progress, and collaborate with an interdisciplinary team to promote independence and safe daily living. HomeHealth HomeBase experience preferred; 1 year of OT experience preferred.

Qualifications

• Current and valid Occupational Therapy license
• Minimum of 1 year of occupational therapy experience preferred
• Home health experience preferred
• Current CPR certification
• Valid driver's license, reliable transportation, and current auto insurance
• HomeCare HomeBase experience preferred

Responsibilities

• Perform comprehensive clinical assessments to identify patient needs and determine appropriateness for occupational therapy services
• Develop, implement, revise, and complete individualized Plans of Care in a timely manner
• Document evaluations, progress notes, physician orders, and all clinical records accurately and efficiently
• Provide therapeutic interventions to improve strength, mobility, coordination, functional independence, and safety
• Assess activities of daily living (ADLs), self-care, work, leisure, and cognitive-perceptual functioning
• Collaborate with IDG to coordinate patient care and outcomes
• Maintain infection control practices and standard precautions
• Communicate with patients, families, physicians, and care team members
• Participate in continuing education and maintain licensing compliance
