Job summary

BAYADA Home Health Care in Lincolnton, NC is seeking an experienced Occupational Therapist (OT) for a per diem to part-time home health role serving adult and geriatric patients in Salisbury Senior Living offices and surrounding communities.

You will join a multidisciplinary team, perform home visits, evaluate function, tailor care plans, and educate families while maintaining high standards of compassionate, reliable care and professional growth opportunities.

Qualifications

• North Carolina OT license required.
• Minimum of one year of recent work experience as an Occupational Therapist.
• Graduation from an occupational therapy program accredited jointly by the AMA and AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills; problem solving abilities.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients’ functional status, including ADL/IADL performance.
• Assess cognitive, psychological and motor aspects to develop intervention plans.
• Evaluate home environments for hazards and barriers to independence.
• Identify equipment needs and environmental adaptations to improve outcomes.
• Collaborate with nursing and the multidisciplinary team on care plans.
• Educate patients and families in rehabilitative care and activities.
• Document observations and interventions accurately using an EMR system.
