Job summary

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for home health visits in Northumberland County. This per-visit position can be per diem, part-time, or full-time, offering flexibility to support adult and geriatric clients in their homes.

As a key member of a multidisciplinary team, you will evaluate functional status, develop individualized plans, educate patients and families, and document care using EMR.

Qualifications

• Pennsylvania OT license required.
• Minimum one year of OT experience.
• Home health experience preferred.
• Graduation from an accredited OT program.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Evaluate functional status and ADL/IADL performance.
• Develop and revise individualized intervention plans.
• Evaluate home environment for hazards or barriers.
• Identify equipment needs and environmental adaptations.
• Document observations and interventions using EMR.
