Job summary

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for a per diem to part-time home health opportunity in Forsyth County and surrounding areas of North Carolina.

You will perform home visits, evaluate functional status and cognition, and develop individualized intervention plans for adult and geriatric clients, collaborating with a multidisciplinary team to promote safe, independent living.

Qualifications

• Requires North Carolina Occupational Therapist license.
• Prior home health experience preferred but not required.
• Minimum of 1 year as an OT.
• Graduation from an ACHE/AOTA-accredited OT program.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Evaluate functional status and develop individualized intervention plans.
• Assess home environment for hazards and adapt as needed.
• Educate clients and families on rehabilitative care and safety at home.
• Document observations and interventions in EMR.
• Collaborate with nursing within multidisciplinary care plans.
