Job summary

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for per diem to part-time home health visits in Forsyth County, NC. You will work with adults and geriatrics across North Carolina in clients’ homes, performing evaluations and developing interventions to promote independence.

Qualifications

• Current North Carolina OT license.
• Minimum of one year of OT work experience.
• Graduate from an OT program accredited by AMA/AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills; problem solving.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Evaluate patients’ functional status and ADL/IADL performance; document findings.
• Assess cognitive, psychological and motor aspects and develop intervention plans.
• Evaluate home environment for hazards or barriers to independence.
• Educate patients and families in rehabilitation to promote safety and independence.
• Document observations and evaluations using EMR on a touch pad tablet.
