Job summary

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for a per diem to part-time opportunity to perform home health visits in Forsyth County and surrounding areas in North Carolina. You will work with adult and geriatric patients within a multidisciplinary team to deliver rehabilitative care that enables home living.

Responsibilities include conducting comprehensive OT evaluations, developing customized intervention plans, adapting home environments, educating patients

Qualifications

• Active North Carolina OT license required.
• One year of occupational therapy experience preferred.
• Graduation from an accredited OT program is required.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills; problem solving abilities.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform evaluations of patients’ functional status including ADL/IADL performance.
• Develop and revise individualized OT intervention plans to achieve outcomes.
• Assess home environment for hazards and barriers to independence.
• Identify equipment needs and environmental adaptations.
• Collaborate with nursing and the multidisciplinary team on care plans.
• Educate clients and families on rehabilitative care and safe home activities.
• Document observations and interventions using EMR on a tablet.
