Job summary

BAYADA Home Health Care in Asheboro, NC is seeking an experienced Occupational Therapist (OT) for a PRN position to perform home health visits with adult and geriatric clients in community settings.

As a valued member of a multidisciplinary team, you will evaluate patients, develop individualized plans, educate families, and document care using EMR.

Qualifications

• Current North Carolina OT license.
• At least 1 year of OT work experience.

Responsibilities

• Make home visits to Clients in designated geographic territory.
• Evaluate functional status including ADLs/IADLs.
• Develop and adjust individualized OT plans to achieve outcomes.
• Assess home environment for hazards and independence.
• Identify equipment needs and environmental adaptations.
• Document observations and interventions in EMR.
