Job summary

Qualifications

• Must have active OT license in Ohio.
• Minimum of 1 year OT experience; home care preferred.
• Excellent communication with patients and families.

Responsibilities

• Provides skilled occupational therapy in patients' homes as ordered by the attending physician.
• Evaluates patient functioning and applies diagnostic/prognostic procedures.
• Develops individualized OT plan of care and adjusts as needed.
• Maintains documentation and complies with Medicare/Federal guidelines.
• Promotes continuity of care and reduces hospital readmissions.
