Job summary

Millennium Home Care, affiliated with Millennium Healthcare, LLC, is seeking a full-time Occupational Therapist to deliver in-home services in Florida. You will perform patient assessments, develop treatment plans, provide interventions to improve daily living skills, and educate families.

The role requires Florida OT licensure, at least 6 months hospital/rehab experience, strong communication, and reliable transportation to visit patients in their homes.

Qualifications

• Active Florida Occupational Therapist license.
• Minimum 6 months experience in a hospital, SNF, or rehab.
• Reliable transportation to visit patients at home.
• Strong communication and professionalism.

Responsibilities

• Conduct comprehensive assessments of patients' physical, cognitive, and functional abilities to develop individualized treatment plans.
• Provide hands-on therapy to improve patients' daily living skills, mobility, and independence. Implement therapeutic interventions, including therapeutic exercises, adaptive equipment training, and environmental modifications.
• Educate patients and families on strategies for managing and adapting to their conditions, maximizing their independence and quality of life.
• Document patient progress, treatment plans, and outcomes accurately and timely.
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care and ensure optimal outcomes.
• Stay up-to-date with the latest research and advancements in occupational therapy practice.
