Job summary

Pinnacle Home Care is seeking Occupational Therapists (OT) to provide compassionate healthcare to patients in their homes across Webster, Center Hill, and surrounding areas. As Florida’s largest Medicare-certified home health provider, we have been delivering high-quality, patient-centered care for over two decades.

The ideal candidate will be responsible for assessing patients’ functional status, developing care plans, and ensuring coordinated care. This role requires a current OT license, excellent communication skills, and a minimum of two years of OT experience.

Qualifications

• Must have a current OT license (state-specific).
• Graduate of an accredited occupational therapy program recognized by the American Medical Association.
• Minimum two years of OT experience (home health experience preferred).

Responsibilities

• Assess the patient’s functional status.
• Evaluate the home environment for hazards or barriers to independent living.
• Collaborate in developing a comprehensive care plan.
