Job summary

Boost Home Healthcare in Michigan is seeking an Occupational Therapist to deliver skilled therapy in patients' homes on an intermittent basis under physician orders and a care plan.

You will assess, plan, and modify therapy, re-evaluate every 30 days, and collaborate with physicians to adjust treatment. A state license and eligibility for national registration are required, with acute care and home health experience preferred.

Qualifications

• Graduate of an Occupational Therapy curriculum accredited by the relevant boards.
• Licensed in the state where practicing (OT).
• Eligible for the National Registration Examination (AOTA/NBA).
• Minimum of one year in an acute care setting.

Responsibilities

• Provide prescribed occupational therapy services to patients in their residence on an intermittent basis.
• Assess, plan, maintain, and modify care; reassessments every 30 days.
• Consult with physicians regarding changes in treatment and outcome goals.

Education

OT degree or equivalent

Licensed in practicing state

Eligible for National Registration Examination

Acute care experience

Home Health experience preferred
