You will collaborate with physicians and care teams to arrange follow-up care and educate patients on lifestyle changes to support mobility and independence. The ideal candidate has a compassionate style of care, strong communication, and the ability to handle a diverse caseload with multiple patients

Qualifications

• Must hold the required state license for occupational therapy.

Responsibilities

• Meet with patients from diverse backgrounds and discuss health concerns and therapy needs.
• Conduct evaluations to determine a starting point for each patient.

• Collaborate with other care team members for follow-up care.
• Maintain accurate records of patient therapy and progress.
