Job summary

Concierge Home Care is seeking a Home Health Occupational Therapist (OTR) to provide 1:1 in-home evaluations and treatment in Nassau County, FL. The role offers a flexible schedule, autonomy, and pay-per-visit options, with full-time or PRN commitments.

You will develop treatment plans, educate patients and families, and collaborate with physicians and the care team. LIC/CPR and driving are required; home health experience preferred.

Qualifications

• Active, unencumbered Florida Occupational Therapist license.
• Valid driver’s license with reliable transportation.
• CPR certification.
• Ability to supervise COTAs.
• Home health experience preferred, but not required.

Responsibilities

• Conduct patient evaluations and home environment assessments.
• Develop individualized treatment plans based on physician orders.
• Provide therapy focused on improving functional mobility and daily living skills.
• Educate patients and caregivers on adaptive strategies and safety techniques.
• Collaborate with physicians and interdisciplinary care teams.
• Supervise and delegate care to COTAs when appropriate.
