Job summary

Concierge Home Care in Saint Augustine, FL is seeking a Home Health Occupational Therapist (OT) for a flexible, full-time role with a earnings potential of $110K+ annually. You will provide 1:1 patient care in the home, develop individualized treatment plans, and work with a supportive team and advanced EMR tools.

The position offers autonomy, minimal weekends, and quarterly bonus opportunities, with mileage reimbursement and a data plan stipend.

Qualifications

• Current Florida OT license is required.
• Driver’s license and reliable transportation.
• CPR certification required.
• Ability to supervise COTAs.
• Home health experience preferred.

Responsibilities

• Evaluate patients and develop individualized home-based OT plans.
• Perform home safety assessments and recommend modifications.
• Provide therapy to improve ADLs and mobility.
• Educate patients and caregivers on adaptive techniques.
• Collaborate with physicians and interdisciplinary teams.
• Supervise COTAs and complete documentation using EMR.
