Concierge Home Care is hiring Home Health Occupational Therapists (OT) for a flexible schedule in Melbourne, FL. Enjoy 1:1 patient care in the home with autonomy, coverage area in Brevard County, and pay-per-visit compensation including Medicare differential pay.

Earn $70–$75 per visit with additional earnings potential, plus optional modified full-time, PRN, or 1099 contractor opportunities. AI-assisted tools and WellSky/Kinnser EMR support efficient documentation.

Qualifications

• Evaluate patients and develop individualized treatment plans.
• Perform home safety assessments and recommend modifications.
• Provide therapy to improve ADLs, functional mobility, and independence.
• Educate patients and caregivers on adaptive techniques and equipment.
• Collaborate with physicians and interdisciplinary teams.
• Supervise COTAs.
• Complete documentation using streamlined technology.
