Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provides service within the scope of practice as defined by state laws and plan of care, coordinating with health care team.
• Evaluates functional status and CMS documentation, supports physician in therapy plan of care.
• Observes, records, and reports patient response and changes to condition.
• Conducts patient assessments of function using appropriate tests.
• Function in diverse home settings respecting all patients' backgrounds.
