Job summary

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Ability to function in any home situation regardless of client demographics.
• Demonstrated ability to coordinate with health care team.

Responsibilities

• Assess patient functional status and ADLs, fine motor, and home assessments.
• Develop therapy plan of care in consultation with physician.
• Document patient progress and respond to changes in condition.
• Conduct patient assessments using functional ability tests.
