Job summary

ABL Health Care LLC is seeking a compassionate Occupational Therapist (OT) for its Home Health team in Lafayette. The OT will evaluate patients, develop and implement care plans, and monitor progress to maintain high standards of care, working with the Clinical Director and staff both in the field and remotely as needed.

The role requires a California OT license, at least 1 year in home health or hospice, and a valid CA driver's license with regular travel within a 15–25 mile radius.

Qualifications

• Must hold a current OT license to practice in California.
• Minimum 1 year of experience in home health or hospice.
• Valid CA Driver's License and ability to travel 15–25 miles regularly.

Responsibilities

• Provide, coordinate and oversee skilled intermittent care to patients in the home.
• Regularly evaluate and re-evaluate patient status.
• Administer treatments, monitor conditions, and report changes to the physician/case manager.
• Develop and revise the patient's plan of care (POC).
• Maintain accurate clinical records and documentation using company device.
