Job summary

ABL Health Care, LLC is seeking a compassionate Occupational Therapist (OT) for a full-time Home Health role in California. The OT will assess patients in their homes, develop and implement care plans, and monitor progress alongside a clinical director and field staff.

This position offers opportunities to travel within the service area and work closely with a dedicated home health team. The ideal candidate will hold a California OT license, have at least 1 year of Home Health or Hospice

Qualifications

• Must hold a California OT license to practice.
• 1 year of experience in Home Health or Hospice is required.
• Valid California Driver's License is required.
• Ability to travel within a 15–25 mile radius on a regular basis.

Responsibilities

• Provide skilled intermittent OT services in patients' homes.
• Evaluate and re-evaluate patient status to adjust care plans.
• Administer therapies and monitor patient conditions, reporting changes as needed.
