Job summary

ABL Health Care in Concord, CA is seeking a full-time Occupational Therapist (OT) for our Home Health team. You will assess patients, create and implement care plans, and monitor progress, working with our Clinical Director and staff in the field.

The role requires an active California OT license, experience in home health or hospice, and a valid CA driver's license. Travel within a 15-25 mile radius is expected. Competitive benefits are offered to full-time employees.

Qualifications

• Current OT License to practice in California.
• Experience in Home Health or Hospice care (1 year recommended).
• Current CA Driver's License.
• Able to travel within a 15 - 25 mile radius on a regular basis.

Responsibilities

• Provide, coordinate and oversee skilled intermittent care to patients in the Home Health setting.
• Regularly evaluates and reevaluates patient status.
• Administer treatments, monitor patient conditions, and report changes to the care team.
• Initiate, develop and revise the patient’s plan of care (POC).
• Educate patient, family and caregiver on condition and care needs.
• Maintain accurate clinical records using company device and comply with policies.
