CARE FIRST HOME HEALTH CARE INC in Riverside, CA is seeking an Occupational Therapist to implement standards of care for home health OT and work under the supervision of the Clinical Supervisor.

The role involves evaluating patient function, developing treatment plans, educating patients and families, maintaining records, and coordinating with physicians to ensure safe, effective rehab in the home setting.

Qualifications

• Graduate of an occupational therapy curriculum accredited by the relevant US agencies.
• Registered with the national certification exam for occupational therapy.
• Licensed as an occupational therapist in California.
• Minimum two years of clinical OT experience.
• Good verbal and written communication and organizational skills.
• Current CPR certification.
• Valid driver’s license and reliable automobile.

Responsibilities

• Evaluates patient functional status and home environment for safety.
• Develops treatment program and goals for improved function.
• Communicates plan of care to physician and team.
• May train patient in ADLs and retrain daily activities as needed.
• May fabricate splints and instruct use of adaptive equipment.
• Trains patient in prosthetic/orthotic device use as applicable.
• Maintains clinical records and reports changes to physician.
• Maintains coordination with other care personnel.
• Supervises OT assistants according to policy and regulations.
• In cases where therapy is sole skilled service, supervises home health aide visits biweekly.
• Maintains patient confidentiality and adheres to organizational policies.
