Job summary

CARE FIRST HOME HEALTH CARE INC is seeking an Occupational Therapist to support home health patients in the Los Angeles area. The OT will implement standards of care, assess patients, and develop plans of care in collaboration with supervising clinicians.

The role includes evaluating functional status, creating treatment plans, and coordinating with physicians and other professionals to promote optimal patient outcomes. Strong documentation, communication, and CPR certification are essential.

Qualifications

• Graduate of an occupational therapy curriculum accredited jointly by the AMA/ACATE and AOTA.
• Registered with the NBCOT National Registration Examination.
• Licensed as an occupational therapist by the California Board of Occupational Therapy.
• Two years of clinical experience as an occupational therapist.
• Demonstrates good verbal and written communication, and organization skills.
• Possess and maintains current CPR Certification.
• Licensed driver with automobile that is in good working order and insured.

Responsibilities

• Evaluates patient’s functional status and home environment for hazards.
• Develops treatment program and communicates plan of care to physicians and team.
• May teach new skills or retrain daily activities as needed.
• May fabricate splints and instruct on adaptive equipment use to improve function.
• Maintains clinical records and reports changes in patient condition.
• Supervises occupational therapy assistants per policy and state regs.
• Coordinates with home health staff to ensure coordinated care.
• Instructs and evaluates home health aide care when therapy is sole skilled service.
• Maintains patient confidentiality; may perform other duties as needed.

Accredited OT curriculum

Registered with NBCOT (OTR)

Licensed by CA Board of OT

Two years clinical OT experience

Verbal and written communication
