Job summary

Atrinity Home Health, part of the LHC Group and Optum family, is seeking an Occupational Therapist to assess and address patients\' functional needs in home health settings. You\'ll evaluate ADLs, motor coordination, home safety, and adaptive equipment as part of a comprehensive care plan.

Responsibilities include coordinating with physicians, documenting CMS-related data, and delivering evidence-based therapy to diverse patients in their homes.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver\'s license and vehicle insurance or access to dependable transportation.
• CPR certification preferred or able to obtain within 90 days of hire.
• Demonstrated ability to manage multiple tasks simultaneously.
• Demonstrated ability to work independently.
• Good communication, writing, and organizational skills.

Responsibilities

• Provides service within the scope of practice as defined by state laws governing Occupational Therapy.
• Evaluates patient functional status and CF CMS documentation; collaborates with physician on plan of care.
• Observes and reports patient response to treatment to nurse supervisor and physician.
• Conducts patient assessments evaluating functional level using standard tests.
